Completed forms may be emailed to [email protected]; faxed to (540)254-2765; mailed to James River High School, 9906 Springwood Rd, Buchanan, VA 24066; or dropped off at the school. (Please call (540) 254-1121 first to make sure someone is available to retrieve the forms.) Should you need printed enrollment forms, please call (540) 254-1121 to arrange a time to pick up a packet.
Once the enrollment papers are received, a school counselor or other office personnel will contact you to schedule an appointment. In addition to the 5 completed forms (above), you will need to provide the following information at your enrollment appointment:
- Document(s) to verify Proof of Residency information:
- Copy of a mortgage, lease, or deed of your principle residence in Botetourt County OR
- If you do not have any of the above, you must provide both of the following:
- current utility bill, AND
- current bank statement or check stub issued by an employer.
Other information you may be asked to provide includes:
- Birth Certificate
- Withdrawal Form from the previous school, if applicable
- An unofficial transcript or final report card from previous school
- Immunization Records
- IEP or 504 (if applicable)
- Custody papers (if applicable)
